STRiVE is governed by a voluntary Board of Directors composed of consumers and family members, as well as human service and business professionals. STRiVE is funded primarily (80%) through the Colorado Department of Human Services and fees from disability payments, with supplemental funding from Mesa County, private donors and grants.

The Case Manager implements the Targeted Case Management (TCM) process for assigned individuals receiving services through one of the waiver programs. Targeted Case Management includes assessment, service plan development, monitoring, referral and related activities. The Case Manager assures provision of quality services to assigned individuals consistent with the individual’s stated goals and desires, MDS policies and prevailing statutes and regulations. The Case Manager completes documentation and billing for all assigned activities in a timely and professional manner.
•Bachelor’s degree from an accredited college or university with a preferred major in Human Services, Social Sciences, Counseling, Sociology or Psychology.
•Valid driver’s license and a driving record that meets MDS standards
•Personal vehicle available for work-related travel
•Minimum of one-year case management experience preferred.
